If your notice of cancellation of basic health insurance arrives after 30 November, it only takes effect on the next date: with a CHF 300 deductible in the standard model, that is 1 July (receipt by 31 March), otherwise the next 1 January. Until 31 December, you can still choose a higher deductible with your insurer; you can switch to a GP, HMO or telemedicine model at any time.

Key points at a glance
- According to the FOPH, a notice of cancellation that is not on time takes effect on the next possible cancellation date; what counts is receipt by the health insurer, not the postmark (Art. 7 KVG).
- With the standard deductible in the standard model, you can switch on 1 July; the notice of cancellation must arrive by 31 March (Art. 7 para. 1 KVG, Art. 94 para. 2 KVV).
- You choose a higher deductible without a statutory notice period with effect from 1 January; according to the FOPH, written notification by 31 December is sufficient (Art. 94 para. 1 KVV).
- You can switch from free choice of physicians to a GP, HMO or telemedicine model at any time (Art. 100 para. 2 KVV).
- If you and your health insurer disagree about receipt, you can request a written decision and lodge an objection against it within 30 days (Art. 49 and 52 ATSG).
Too late means: on the next date
For switching on 1 January, your notice of cancellation of basic health insurance must reach the health insurer by 30 November (Art. 7 para. 2 of the Federal Health Insurance Act, KVG). If it arrives later, it is not invalid, but it only takes effect later: according to the FOPH, a notice of cancellation that is not on time takes effect on the next possible cancellation date. Which date that is depends on your insurance:
| Your basic health insurance | Next switching date | Notice of cancellation must arrive by | Legal basis |
|---|---|---|---|
| Standard model, CHF 300 deductible (children CHF 0) | 01.07.2027 | Wednesday, 31.03.2027 | Art. 7 para. 1 KVG |
| Optional deductible, GP, HMO or telemedicine model, bonus insurance | 01.01.2028 | 30.11.2027 or after the premium notification | Art. 7 para. 2 KVG, Art. 94 para. 2, 97 para. 2, 100 para. 3 KVV |
Even so, the year is not entirely lost. With your own health insurer, you can still change some aspects of your deductible and model, and in some cases it is worth checking whether the notice of cancellation really was too late.
Step by step: what to do now
- Check whether the notice of cancellation really was too late. What counts is receipt by the health insurer during normal business hours, not the postmark. Look in the tracking to see when the letter was delivered. According to the FOPH, a registered letter in the health insurer’s PO box is only deemed delivered when it is collected; if it was only collected on 1 December, it was too late.
- Ask the health insurer for which date it is recording the cancellation. Ask for a written reply. It is also your confirmation of cancellation for the later date.
- Inform the new health insurer. If you have already registered, tell it immediately that your notice of cancellation arrived too late. Your old basic health insurance only ends once the cancellation takes effect and the new insurer has reported uninterrupted cover (Art. 7 para. 5 KVG). What applies with two registrations is explained in the article Double insured.
- Save with your own health insurer where you still can. A higher deductible by 31 December, a model with restricted choice of doctor at any time. Details in the next section.
- Note the next date. In the standard model with a CHF 300 deductible: notice of cancellation to arrive by 31.03.2027. Otherwise: wait for the premium letter in autumn 2027, notice of cancellation to arrive by 30.11.2027.
The deadline calculator works out the next possible date for your situation:
What is still possible with your own health insurer
The 30 November deadline for switching applies to changing health insurer, to a lower deductible and to leaving a model. Other changes with your own health insurer do not require a notice period:
| Change | By when | Effective | Legal basis |
|---|---|---|---|
| higher deductible | according to the FOPH, in writing by 31 December | 1 January | Art. 94 para. 1 KVV |
| from free choice of physicians to a GP, HMO or telemedicine model | at any time | according to the FOPH, at any time during the year | Art. 100 para. 2 KVV |
| into bonus insurance | no statutory notice period | 1 January | Art. 97 para. 1 KVV |
| lower deductible, leaving a model or bonus insurance | receipt by 30 November | 1 January | Art. 7 para. 2 KVG, Art. 94, 97, 100 KVV |
How to change your deductible and which levels there are is explained in the glossary term Changing your deductible. A higher deductible lowers the premium, but you bear more costs yourself. The discount is limited: it may not exceed 70 % of the additional risk taken on (Art. 95 para. 2bis of the Health Insurance Ordinance, KVV). From CHF 300 to CHF 2,500, that is at most CHF 1,540 a year. Whether this pays off for you depends on your costs; the deductible calculator shows the break-even point. This calculation is not a recommendation.
If the health insurer disputes receipt
If, according to your records, your notice of cancellation arrived in time but the health insurer says it was too late or never arrived, what counts is proof. You have to prove timely receipt, because you derive rights from the cancellation (Art. 8 of the Swiss Civil Code, CC). Good proof is the tracking of a registered letter or A Mail Plus and a confirmation of receipt from the health insurer. How to get reliable proof next time is explained in the article Cancelling by email or by registered mail?.
If the health insurer maintains its position, you can request a written decision: the health insurer must issue a written decision on orders you do not agree with (Art. 49 para. 1 of the Federal Act on the General Part of Social Insurance Law, ATSG). You can lodge an objection against the decision with the health insurer within 30 days; the objection procedure is free of charge (Art. 52 ATSG). Beforehand or in parallel, the Office of the health insurance ombudsperson gives free advice and mediates in problems with switching health insurer.
What another year can cost: premiums 2026
How much it makes to stay with an expensive health insurer for another year is shown by the 2026 premiums in the municipality of Basel (region 0), adults born in 1988, with accident cover, across all 117 offers and models (source: FOPH):
| Monthly premium 2026 | Deductible CHF 300 | Deductible CHF 2,500 |
|---|---|---|
| cheapest offer | CHF 575.90 | CHF 447.60 |
| median of all offers | CHF 637.00 | CHF 508.70 |
| most expensive offer | CHF 809.70 | CHF 681.70 |
With a CHF 300 deductible, there was a difference of CHF 2,805.60 a year between the most expensive and the cheapest offer, and CHF 733.20 between the median and the cheapest. For the most expensive offer, a standard model, the premium with a CHF 2,500 deductible fell by CHF 128.00 a month or CHF 1,536.00 a year, just below the statutory cap of CHF 1,540. The FOPH publishes next year’s premiums at the end of September; you can calculate with them in the premium calculator.
Example: arrived on 1 December
Example with fictitious people. Tim, 38, and his sister Nora, 35, live in Basel. Both send their notice of cancellation on Monday, 30.11.2026, by A Mail Plus. For both, the tracking shows delivery on Tuesday, 01.12.2026.
| Tim | Nora | |
|---|---|---|
| Basic health insurance | standard model, CHF 300 deductible | telemedicine model, CHF 1,500 deductible |
| Cancellation takes effect on | 30.06.2027 | 31.12.2027 |
| What is still possible | registration with the new health insurer for 01.07.2027 | notify a higher deductible by 31.12.2026, plan to switch for 2028 |
| Next key date | notice of cancellation to arrive by 31.03.2027 | receipt by 30.11.2027 |
Special cases
- Supplementary insurance cancelled too late. The supplementary insurance then continues until the next date under your contract. The law allows ordinary cancellation at the end of the third or any subsequent year with three months’ notice (Art. 35a of the Insurance Contract Act, ICA); the exact dates are in the general terms and conditions. You can only cancel at any time for good cause, if continuing the contract cannot reasonably be expected of you in good faith (Art. 35b ICA). The general terms and conditions and the information from the health insurer prevail.
- Premium change during the year. If, exceptionally, the health insurer notifies you of a new premium during the year, you can cancel again with one month’s notice (Art. 7 para. 2 KVG).
- Moving or changing jobs. If you have to leave your health insurer because you move your place of residence or change jobs, the insurance ends when you move or start the job (Art. 7 para. 3 KVG). According to the FOPH, however, a higher premium because of a move does not give you an extraordinary right of cancellation.
- Unpaid invoices. If your notice of cancellation was on time but arrears for which you received a reminder have not been paid by 31 December, it does not take effect either; you can only switch on the next date.
Common mistakes
- Hoping for goodwill and doing nothing. The deadlines apply equally to all health insurers. Clarify in writing on which date the cancellation takes effect, and plan accordingly.
- Not informing the new health insurer. A registration that cannot take effect leads to queries and duplicate post. Let the new health insurer know immediately.
- Switching hastily from the standard model to the telemedicine model. You lose the 1 July date.
- Notifying a higher deductible after 31 December. It then only takes effect a year later.
- Throwing away the proof. Without tracking or a confirmation of receipt, you cannot prove timely receipt.
How to send your next notice of cancellation safely is shown in the article Cancelling by email or by registered mail?, and all deadlines are under Health insurance notice periods. The cancellation letter generator creates a letter for the next date, and Switching health insurer shows the whole procedure. You will find further articles in the Switching health insurer section.
Frequently asked questions
What happens if my notice of cancellation arrives on 1 December?
Then it is too late for switching on 1 January. According to the FOPH, it takes effect on the next possible date: on 1 July with a CHF 300 deductible in the standard model, otherwise on the next 1 January. Ask your health insurer for which date it is recording the cancellation.
Can I have the deadline extended?
No. The deadlines for basic health insurance are set out in Art. 7 KVG and apply equally to all health insurers. If 30 November falls on a weekend or public holiday, according to the FOPH the deadline ends on the last working day before it, not after it.
I have already registered with the new health insurer. What now?
Inform the new health insurer immediately that your notice of cancellation arrived too late. Your old basic health insurance only ends once the cancellation takes effect and the new insurer has reported uninterrupted cover (Art. 7 para. 5 KVG). There is no such thing as double basic health insurance.
Can I still save on premiums?
Yes, with your own health insurer. According to the FOPH, you can choose a higher deductible until 31 December with effect from 1 January, and you can switch to a GP, HMO or telemedicine model at any time. The deductible calculator and premium calculator show whether this pays off for you; this is not a recommendation.
The health insurer says my notice of cancellation never arrived. What should I do?
Present your proof, such as the tracking of the registered letter. If the health insurer maintains its position, you can request a written decision and lodge an objection free of charge within 30 days (Art. 49 and 52 ATSG). The Office of the health insurance ombudsperson gives advice free of charge.
